We help people learn how to navigate the challenges that come with a diagnosis

Whether you’ve been diagnosed, are caring for someone with dementia, or simply want to learn more about it, the Alzheimer Society is the first place to turn for help and support.

First Link directs you to dementia-related health services, support and information near you. For example, you can:

  • receive one-on-one or group support services
  • get connected to local health care providers and community services
  • meet other people in similar circumstances and exchange experiences
  • get help planning your future and managing the disease
  • benefit from stimulating activities if you’re a person living with dementia, and respite if you are a care partner.

How does the referral program work?

The aim of referral is to give people diagnosed with dementia the easiest possible access to services, information and care.

Health, legal and financial professionals, as well as community organizations, can refer a care partner of a person living with dementia to the services offered by an Alzheimer Society by completing a brief online form at https://referenceaidancequebec.ca/en. We will contact the care partner within 10 business days and provide the needed information, support, education or respite.

Note that a care partner can be referred at any stage of the person’s illness, even during screening prior to the specific diagnosis.

This initial conversation is the first step towards your First Link support.

 

Who can refer me?

You can be referred to an Alzheimer Society by any health or social services professional in your area, including:

  • your family doctor
  • a medical specialist 
  • a nurse
  • a social worker
  • a pharmacist
  • an occupational therapist
  • a legal or financial professional
  • a community organization worker.
