SCREEN: Measuring nutrition risk in community-living seniors

What is this Study about?

We want to determine if a nutrition questionnaire (SCREEN) works well to identify nutrition challenges for older adults with diagnosed memory problems, cognitive impairment or dementia when completed by themselves, or by a care partner on behalf of their cared for person.

What Will Happen in This Study?

Three study visits will occur at your home or at the University of Waterloo.

Visit 1 (1 hour): questionnaires, physical and functional assessments will be completed. A nutrition questionnaire will be administered. You will be asked to record your food and beverage intake for one day.

Visit 2 (30-45 minutes): we will review your food record. The nutrition questionnaire will be completed for a second time.

Visit 3 (1 hour): a dietitian will complete a nutrition assessment. Individualized tips to improve food intake and nutrition will be provided.

A $25 gift card or equal value gift is provided for participation.
