Strategic Plan

As the number of people living with dementia continues to grow, the need for accessible, responsive, and innovative support services has never been greater. Our 2026–2031 Strategic Plan reflects our commitment to meeting this evolving need through collaboration, community engagement, and forward-thinking approaches to care.

Our Commitment to Care, Community and Connection

Developed through extensive consultation with clients, care partners, volunteers, staff, community partners, and leadership, this plan outlines a shared vision for the future—one that strengthens access to services, promotes greater awareness and understanding of dementia, and advances more equitable and integrated approaches to care. Over the next five years, we will focus on enhancing the client experience, deepening partnerships across the health and community sectors, and fostering dementia-friendly communities where everyone can thrive. Together, we are building a future that supports better care, stronger connections, and meaningful collaboration for all those impacted by dementia.

We look forward to sharing our progress and introducing new initiatives that will help shape the future of dementia care and support throughout Durham Region!
