Posting: Behaviour Supports Ontario (BSO) Counsellor 

Job Type: Full-time, permanent – 35 hours/week 

Job Description: The BSO Counsellor provides expertise to support an interdisciplinary team that works with persons with behavioural challenges living in their own homes, in the community, hospital and in long-term care homes, as well as their families and care providers. 

Working closely with the hospital based BSO Mobile Support Team, and other allied health care providers, the BSO Counsellor will contribute to improving the quality of life for clients experiencing responsive behaviours related to dementia. Their goal is to build community capacity to support persons with a diagnosis, their care partners, or allied health professionals in the place where people live. The BSO Counsellor supports a team that provides behavioural support to individual clients and caregivers, transitional management between health care providers, and ongoing training and mentorship for service provider agency staff, clinicians, and families. 

Qualifications: 

 - University degree in Social Work, and/or other registered professions 

 - Current registration with Ontario College of Social Workers and Social Service Workers, or other health-related professional college 

 - Prior experience with elderly clients in community and/or long-term care settings 

 - Training in P.I.E.C.E.S, U-First! and GPA an asset 

 - Demonstrated extensive knowledge and skill working with elderly individuals with various types of dementia, delirium, mental health issues, addictions, and other neurological conditions 

 - Demonstrated knowledge of a systems approach to person-centered care, including the effect of responsive behaviours on the person, family members and caregivers 

 - Demonstrated person and caregiver focused care that recognizes the uniqueness, strengths, and abilities of each person, and promotes optimal quality of life 

 - Knowledge of relevant community resources and service systems to facilitate support for clients, family members and caregivers through a process of community integration and advocacy 

 - Flexibility in adapting to changing priorities e.g., to represent the Society at events and programs which occur outside of regular work hours 

 - Provide a systematic and practical framework for assessment in consultation with Mobile team members to formulate diagnostic impressions and develop a holistic plan to address client needs 

 - Advanced communication and interpersonal skills to function effectively within a multidisciplinary team environment serving a variety of clients, their family members, and caregivers. Strong skills in documentation and case-related correspondence  

 - Driver’s license with access to a vehicle 

 - Vulnerable Sector Police Screening

Position Summary: 

The Community Development Coordinator will assist all staff within administrative, programs and events positions. Reception coverage will be required which is the first point of contact for phone calls and visitors.

- Registered in post-secondary programs in Health Sciences, Social Work, Recreation and Leisure, or Marketing and Communications 

- Able to work within an environment that requires a high degree of professionalism, confidentiality, and discretion 

- Excellent interpersonal, communication and organizational abilities 

- Strong administrative skills/social media savvy  

- Ability to work independently and be a cooperative team player 

- Teachable and eager to learn 

- Proficient in computer skills including ability to use electronic communication devices, software pertaining to documentation and publishing and marketing

Specific Job Responsibilities: 

- Administrative tasks (phone reception, filing, typing, copying, scanning, etc.) 

- Reception responsibilities: 

- First point of contact for phone calls and visitors, greeting and welcoming in a professional, friendly manner 

- Uses discretion in referring visitors / calls to the appropriate staff; directs callers and visitors to the appropriate resources as necessary 

- Assists in screening and safety processes, as required 

- Correlation of educational and fundraising materials 

- Data Entry 

- Interact and network with community agencies, businesses, sponsors to promote fundraising and public events 

- Assist with coordinating and implementing of fundraising events and social recreation programs 

- Assist with volunteer coordination (scheduling) and volunteer activities 

- Interact with clients, care partners and volunteers to assist with delivery of programs (i.e., attending support groups, social recreation) 

- May be required to prepare the bank deposit using the appropriate bank deposit forms and take deposits to the bank as per financial policy 

- Initiative to improve and bring new learnings to projects/events 

- Other duties as assigned by the Manager Fund Development, Communications, and Volunteers, and/or Executive Director 

Working Conditions: 

- 35 hours per week for 8-10 weeks (contract dates flexible) 

- Frequent interaction and collaboration with staff, clients, volunteers, donors, and public 

- Frequent sitting, general office duties, typing, filing, etc. 

- Work schedule fairly regular with flexibility depending on event/programs 

- Frequent multi-tasking and deadlines 

- Some physical activity with client groups 

- Some travel required throughout Huron and Perth Counties using own vehicle 

- Occasional lifting/carrying 25 lbs., reaching, bending, walking.

- Assisting the Intake team to ensure that persons diagnosed with dementia and their care partners have timely access to information, learning opportunities and support services

Posting: In-Home Recreation Coordinator

Job Type: Part-time, permanent – 28 hours/week

Job Description: Reporting to the Manager of Fund Development, Communications, and Volunteers, the In-Home Recreation Coordinator will provide clients the opportunity to participate in a range of activities that are stimulating, therapeutic, and socially engaging in their own home.

Qualifications:

· Completed post-secondary education in Therapeutic Recreation and/or Recreation and Leisure Diploma

· Minimum 2 years’ experience working with volunteers

· Minimum 2 years’ experience working with persons with dementia and their care partner

· Ability to work independently and as a cooperative team member

· Strong communication skills

· Demonstrated time management/organizational skills meeting deadlines

· Flexibility in adapting to changing priorities e.g., to represent the Society at events and programs which occur outside of regular work hours

· Valid driver’s license and vehicle essential

· Vulnerable Sector Police Screening

Responsibilities:

· Using a client centred approach, provide information to individuals and families affected by Alzheimer’s disease and other dementias

· Develop care plans for activities with client, family, and volunteer

· Evaluate programs in collaboration with Manager of Fund Development, Communications, and Volunteers

· Maintain a current knowledge of Alzheimer’s disease and other dementias

· Initiate and maintain educational/information sessions in the community about the program and other volunteer opportunities

· Provide leisure education for care partners and other health care professionals involved in client care

· Update In Home Recreation Training as new education, research and knowledge is gained

· Purchase activity items for in-home kits for clients and volunteer visits as needed

· Maintain an inventory list of the in-home recreation kits and recreation items

· Document statistics daily in Client Management Database

· Submit monthly reports and quarterly statistics

· Collaborate, cooperate, and network with the community at large, and Alzheimer Societies in Ontario

· Work closely with First Link Program staff for ongoing assessment of clients for In Home Recreation programs

· Liaise with community partners in conjunction with the program, education, and fund development staff

· Process client referrals and facilitate matching process with appropriate volunteers and family including the Volunteer Coordinator as appropriate

· Create recreation intervention plans for volunteers to execute with matched clients. Note that some more complex clients will be matched with IHR Coordinator

· Use a holistic approach, providing activities in keeping with the individual’s cultural diversity, recreational interests, activities of daily living with the therapeutic recreation domains as the overarching goal

· Serve as a liaison to the families, providing them with information about the Alzheimer Society Huron Perth and its services, and directing them to the appropriate supports

· Provide regular feedback to families (virtually, email, face to face or over the phone)
