Dementia Journey Survey
The Alzheimer Society of Canada, in partnership with the College of Family Physicians of Canada, is asking you to participate in this 20-minute survey to help us understand your experience with the care you receive from your family physician or other healthcare providers.

Your experience is essential for us to design and develop new dementia care resources so family physicians can better support people and their caregivers through the dementia journey.
The Alzheimer Society of Canada will be also be able to create culturally specific dementia tools so we can address the gaps in supports and services for diverse communities.
There are two surveys: one for people living with dementia, and one for caregivers of people with dementia.
Living With Dementia Experience
There are a few ways you can complete the survey:
- Use the online version.
- Download the fillable-pdf, which you can upload anonymously below, or email it back to us at research@alzheimer.ca (your email will be de-identified to ensure anonymity during the evaluation)
- Use the printable version and mail it back to us in our Toronto office (details included in the survey).
Caregiver Experience
As above, you can complete the survey in one of the following ways:

Lead investigators
Ngozi Iroanyah
Ngozi is a doctoral candidate with York University, working to understand the intersection of policy and culture in dementia care. A caregiver herself, Ngozi has extensive experience in participatory research, community and stakeholder management.
Haridos Apostolides
Haridos has worked across various non-profit organizations over the last decade, including British health charities like the National Eczema Society and Muscular Dystrophy UK where he spent time analyzing data and managing projects.
Principle Investigators
Dr. Saskia Sivananthan
Dr. Sivananthan is the Chief Research & Knowledge, Translation & Exchange Officer at the Alzheimer Society of Canada, and an active neuroscientist and health data scientist. Previous, she served as a senior strategy and policy advisor for the World Health Organisation (WHO) on its global dementia strategy.
Dr. Sid Feldman
Dr. Feldman is a practicing Family Physician, the Chief for the Department of Family and Community Medicine at Baycrest Health Sciences, and Associate Professor for Family and Community Medicine at the University of Toronto.
